(1) Vendor needs to provide employee assistance program service to the government authority located in Columbus, NE.
- Provide program implementation training to the district's designated EAP coordinator.
- Provide program orientation to all of the district's supervisory personnel.
- Provide program orientation to all district employees.
- Provide on-site training programs by qualified staff at the district's request.
- Provide pamphlets, wallet cards, brochures, posters and other promotional materials for communication with and distribution to, district employees.
- Provide a unique password to the EAP website for ongoing access to website information, including newsletters, training programs or calendars, educational and promotional materials.
- Provide annual and periodic utilization reports to the district containing, at a minimum, the following information:
- Counselling statistics and rates, including problems reported and number of sessions attended.
- Counselling referrals made to professional or community resources.
- Utilization rates and comparisons.
- Training, consultations, crisis management services, hours, attendance, program titles or service type, etc.
- Client, supervisory referral and training satisfaction participant composite reports.
- Provide consultation with the district on all matters of program services as needed.
(2) All question must be submitted no later than July 27, 2023.
