Government Authority located in Omaha, Nebraska; USA based organization looking for expert vendor for employee drug and alcohol testing and new employee and dot physical examinations services.
[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide employee drug and alcohol testing and new employee and dot physical examinations services including approved collection sites, appropriate collection supplies, initial screen and approved confirmation testing, overnight transportation and appropriate chain of custody forms. Split sample urinalysis drug testing for Amphetamines (including Methamphetamines), Cannabinoids Marijuana/THC), cocaine, opiates, opioids and phencyclidine (PCP) to the government authority located in Omaha, NE.
- Blood-Alcohol test with positive initial screens confirmed by gas chromatography with flame ionization detector.
- Vendor shall be responsible for providing a statistically valid system for the random selection of employees and for randomly selecting employees from a list supplied by the district and for conducting such tests.
(2) All questions must be submitted no later than August 7, 2020.
(3) Contract term will be one year.

- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
