[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide qualified vendors to enter into a contract to provide managed care services for work related injuries and illnesses of county employees.
- Preparation of a Summary Plan Description is to be distributed by the Plan Provider to all present employees and future employees of the County. This will include the printing of the Summary Plan Description. This booklet must include the essential elements of the Program.
- Preparation and distribution of all forms will be done by the Plan Provider and will require their use by medical or other health care providers.
- Review and processing of medical bills incurred by injured County employees and periodic submission of a master billing to the County for mutually agreeable periods and on a mutually agreeable schedule.
- The master billing format should identify the name of the injured employee, social security number, Division/Department, date of injury, diagnostic codes, care providers, dates of service, incurred charges and Nebraska Workers’ Compensation Court medical fee schedule allowable amount and paid amount.
- Provide suitable facilities, personnel, procedures, processes, systems and forms for the
administration of the Program.
- Maintenance of records and accounts necessary to substantiate all billings and charges to the
County
- Provision of case management services for each employee who incurs lost time or ongoing
medical expenses.
(2) All the questions must be submitted no later than June 15, 2021
[C] Eligibility:
- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
